Louisville Asthma Data Innovation Project

The City of Louisville is making asthma a public health priority – are you?

Better manage your asthma by signing up to receive a FREE Asthmapolis sensor that automatically tracks when you use your inhaler and gives you feedback on how to improve your asthma control. 500 people in Louisville can now sign up for Asthmapolis for FREE.

To be eligible, you or your child must have asthma, an iPhone or Android smart phone, and be a full-time resident of Louisville.

The program is brought to you by the City of Louisville and is funded by Norton Healthcare, Foundation for a Healthy Kentucky, and the Owsley Brown Charitable Foundation.

To sign up, fill out the form below and we will send you a FREE sensor for your inhaler in the mail.

The Asthmapolis system is FDA cleared for prescription use only. We believe that your physician is an important part of your asthma management and that engaging with him or her in a partnership is important while using our system. We require that all participants in the Louisville Asthma Data Innovation Project let their physician know they are participating in the program. By signing up, you agree to discuss this directly with your physician.
